EASA Class 2 & LAPL medical certificates
As an Aeromedical Examiner I am able to provide examinations for the issue, re-validation and renewal of medical certificates class 2 and LAPL. These examinations and the medical criteria applied will be performed accordingly to directive EU 1178/2011 (EASA).
In case of an examination for an initial issue, the applicant shall provide:
☐ a valid ID card or passport
☐ a valid social security card number
☐ the “Decrease in medical fitness statement” form
☐ the “Mental Health questionnaire” form
☐ the “Application for a medical” form
☐ a copy of the most recent spectacle prescription.Only if visual correction is needed to
meet the visual requirements.This can be done with an optician of your choice
☐ results of blood and urine analysis.A prescription will be provided beforehand
In case of an examination for a revalidation or renewal, the applicant shall provide:
☐ a valid ID card or passport
☐ a valid social security card number
☐ the “Decrease in medical fitness statement” form
☐ the “Mental Health questionnaire” form
☐ the “Application for a medical” form
☐ a copy of the most recent spectacle prescription.Only if visual correction is needed to
meet the visual requirements.This can be done with an optician of your choice
☐ results of blood and urine analysis.A prescription will be provided beforehand
☐ current Part-FCL license
☐ last Part-MED medical certificate
Please note:
☐ The medical examination has to be done without and with corrective glasses, so please do not wear contact lenses for this examination.
☐ EASA requires random drug and alcohol screening.
